Important Information on
Required Event Form
NOTE: This form is REQUIRED TO PARTICIPATE IN AN AWANA EVENT-NO EXCEPTIONS! Please read this information carefully, and consult with your Commander or Pastor if necessary. If you have any questions, please contact your Event Coordinator several days prior to the event.
While we don't enjoy making more paperwork for clubs, in today's legal environment, these are required for the protection of Awana, your church, you, and most importantly, for our kids. Please help us out and come prepared with these forms signed! Ask your Event Coordinator or Awana Missionary if you have any questions about these forms.
All event participants are required to have a Parent Consent and Release of Liability Form to participate in events. A release from your church is no longer acceptable-we must have one for every participant at each individual event! This form will be turned in at check-in before every Awana event. For example, if you completed one at AwanaGames™, a new form is also required for Bible Quiz, even if it is just a month later. Also, please note that this form only releases Awana Clubs International for the event, not your local church. We HIGHLY encourage you to have a separate one for your church that fits your church's legal requirements.
No names or identifying information will be posted about any child on the territory website. If a parent does not want to sign the media release portion of the form, please clearly mark out that portion with a black marker. At the event, it will be the coach's responsibility to inform the event coordinator or ministry team of the child who cannot be photographed so we do not post their photo on the website.
Event Participation Agreement
For veteran churches, the Event Participation Agreement is no longer required-there is no need to worry about it! For new churches, if you have no clue what this means, rejoice-you have one less form to complete! :-)
